    These Terms of Service ("Terms") govern your access to and use of the Bridge Medical website (www.bridgemedical.co), patient portals, telehealth services, and all related content, features, and functionality (collectively, the "Services"). By using our Services, you agree to be bound by these Terms. If you do not agree, please do not use our Services.

    1. Eligibility & Use of Services

    You must be at least 18 years of age or the legal age of majority in your jurisdiction to use our Services. By using the Services, you represent that you meet this requirement and that all information you provide is accurate, current, and complete.

    Our Services are intended for patients located in the State of Colorado. Telehealth services are only provided to individuals physically located in Colorado at the time of the virtual visit, as required by state licensure laws.

    2. Medical Services & Telehealth Disclaimer

    Bridge Medical provides medical care through both in-person visits and telehealth (virtual) consultations. Telehealth involves the delivery of healthcare services using electronic communications, video conferencing, and other technology.

    Important Limitations of Telehealth:

    • Not all medical conditions can be diagnosed or treated via telehealth. You may be asked to come in for an in-person examination.
    • Telehealth is not appropriate for medical emergencies. If you are experiencing a life-threatening emergency, call 911 or go to the nearest emergency room immediately.
    • Technical failures, including poor internet connection, may disrupt or prevent telehealth services. Bridge Medical is not liable for delays or failures caused by technology issues outside our control.
    • You are responsible for ensuring you have adequate equipment (camera, microphone, internet) and a private, secure location for your telehealth visit.

    3. No Doctor-Patient Relationship from Website Use Alone

    Simply browsing this website, reading content, or submitting an inquiry does not create a doctor-patient relationship. A doctor-patient relationship is established only after you complete intake, are accepted as a patient, and receive care from a licensed provider at Bridge Medical.

    4. Patient Responsibilities

    As a patient using our Services, you agree to:

    • Provide truthful, accurate, and complete health information.
    • Notify us promptly of any changes to your contact information, insurance, or medical history.
    • Follow the treatment plan and instructions provided by your provider.
    • Attend scheduled appointments or provide adequate notice if you need to cancel or reschedule.
    • Use our patient portals, check-in systems, and communication tools only for their intended purposes.
    • Not share your login credentials or patient portal access with others.

    5. Appointment Cancellations & No-Shows

    We require at least 24 hours' notice for appointment cancellations or rescheduling. Repeated no-shows or late cancellations may result in discharge from the practice or additional fees. Controlled substance prescriptions may not be refilled if appointments are missed without notice.

    6. Fees, Billing & Insurance

    Bridge Medical accepts Medicaid and self-pay (cash) patients. Private insurance is accepted only for Spravato (esketamine) treatment, subject to prior authorization and eligibility verification.

    You are responsible for any copays, deductibles, or fees not covered by your insurance. Payment is expected at the time of service unless other arrangements are made in advance.

    7. Controlled Substance Policies

    If you are prescribed controlled substances (e.g., buprenorphine for MAT), you must comply with all federal and state regulations, including:

    • Attending all required in-person visits and urine drug screenings (UDS).
    • Participating in counseling or peer recovery support as directed.
    • Not requesting early refills except in documented emergencies.
    • Not sharing, selling, or diverting prescribed medications.

    Violation of these policies may result in immediate termination of prescribing privileges and discharge from the practice.

    8. Text Messaging & Communications

    Bridge Medical sends text messages only to patients who affirmatively opt in using the dedicated text-message checkbox on our intake form. The checkbox is unchecked by default. Entering a phone number alone does not enroll you in text messaging. Messages are limited to appointment confirmations, appointment reminders, prescription pickup notices, and replies to messages you send us. Message frequency varies. Message and data rates may apply. Consent is not a condition of receiving care. Reply STOP at any time to opt out, or HELP for help. See our Privacy Policy for details.

    9. Intellectual Property

    All content on this website, including text, graphics, logos, images, and software, is the property of Bridge Medical or its licensors and is protected by copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works without our express written permission.

    10. Prohibited Conduct

    You may not use our Services to:

    • Impersonate any person or entity.
    • Transmit malware, viruses, or harmful code.
    • Attempt to gain unauthorized access to our systems or patient data.
    • Harass, threaten, or abuse staff, providers, or other patients.
    • Use automated scripts, bots, or scrapers to access the website.
    • Post or transmit unlawful, defamatory, or obscene content.

    We reserve the right to terminate access and report illegal activity to law enforcement.

    11. Limitation of Liability

    To the fullest extent permitted by law, Bridge Medical, its providers, employees, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of the Services, even if advised of the possibility of such damages.

    Our total liability for any claim arising from these Terms or the Services shall not exceed the amount you paid for the specific service giving rise to the claim.

    12. Indemnification

    You agree to indemnify and hold harmless Bridge Medical and its officers, employees, agents, and affiliates from any claims, damages, losses, or expenses (including reasonable attorneys' fees) arising out of your violation of these Terms or your use of the Services.

    13. Governing Law & Dispute Resolution

    These Terms are governed by the laws of the State of Colorado, without regard to conflict of law principles. Any dispute arising under these Terms shall be resolved through binding arbitration in Denver, Colorado, except that either party may seek injunctive relief in a court of competent jurisdiction.

    14. Changes to These Terms

    We may update these Terms at any time. Changes will be posted on this page with a revised effective date. Continued use of the Services after changes constitutes acceptance of the updated Terms.
